Forgive me if I've overlooked the obvious, but I've come up empty searching the BOM, the SN Voice website and this forum..

What are part numbers C23, C24, R64, R65 and U5?

Those parts are not on the original layout but have been added to the SMS (the pcb you have) version. You need the documentation for the SMS board. I'll take a stab at attaching it here.
